The University of Kansas Health System's Health Information Management (HIM) team is committed to protecting the privacy and security of your health information. Our team also ensures your medical records are available for continuing care, payment, personal health needs or other uses.
Patients who need to access their records for personal use or continuing care can easily access or request records via MyChart, our dedicated patient portal. MyChart allows you to download, save, print or request copies of self-downloadable health records, including:
To request records that cannot be self-downloaded, log in to MyChart and select Request Medical Record on the main MyChart menu. Fill out the records request form completely and follow the prompts to submit the request.
You can also request your records through the Sharing Hub in MyChart. Click Sharing Hub from the main menu, choose Request Formal Copy of Health Record, then follow the prompts to request your records.

You can also request medical records for personal use or continuing care by completing our Patient-Directed Request form and returning it to Health Information Management.
To obtain a copy of a deceased patient's medical record:
If you are requesting a deceased patient's records for purposes other than settling personal or financial affairs of the decedent, contact Health Information Management, 913-588-2454, option 1, or ROI@kumc.edu.
If you need to send your medical records to a 3rd party, such as an insurance company or attorney, please complete the Patient-Directed Request form and return it to Health Information Management. These requests cannot be completed through MyChart.
